Job summary

HonorHealth seeks an outpatient occupational therapist to join its PRN float pool serving multiple Phoenix-area locations, including Osborn MOB and Deer Valley MOB. The role supports diverse patients with physical disabilities and functional limitations in outpatient settings.

The OT will evaluate patients, develop care plans, provide evidence-based therapies, document progress, and collaborate with physicians, case managers, and therapy staff.

Qualifications

• Bachelor's in Occupational Therapy from an accredited university.
• Master's or Doctorate in Occupational Therapy preferred.
• Arizona OT license in good standing required.
• BLS certification required.

Responsibilities

• Evaluates patients and develops individualized OT treatment plans.
• Provides therapy using activities of daily living and mobility training.
• Documents assessments, progress notes, and discharge plans.
• Collaborates with interdisciplinary team and case managers.
• Supervises OT assistants and participates in rounds.
