Job summary

Hometown Urgent Care in Ohio is seeking a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in a collaborative, team-based setting. You will conduct exams, manage diverse conditions, perform procedures, and coordinate with ENT, surgical and subspecialties, using E-ClinicalWorks EMR.

Strong communication and empathy are essential for quality patient care.

Qualifications

• Ohio license as NP-C/FNP or PA-C required.
• Experience in primary care or internal medicine.
• Ability to manage a broad patient panel across ages.
• Excellent communication and empathy.
• Team-oriented and eager to learn.

Responsibilities

• Perform comprehensive exams and patient histories.
• Diagnose and manage conditions across the lifespan.
• Provide primary care across specialties and procedures.

Job description

We are seeking acompassionate and skilled Advanced Practice Provider (NP or PA)to provide comprehensive primary care services to patients across the lifespan. What You’ll Do:

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists inENT, Surgical, and Medical Subspecialtiesto ensure continuity and coordination of care
• UtilizeE-ClinicalWorksfor efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability toconnectwith patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
