Hometown Urgent Care & Occupational Health is seeking a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in a team-based setting. You’ll diagnose, manage, and educate patients, collaborating with specialists to ensure coordinated care and continuity.

The role requires Ohio licensure (NP-C/FNP or PA-C), outpatient experience, and strong communication.

Qualifications

• Must be licensed NP-C/FNP or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Outpatient care with broad patient panel across ages and backgrounds.
• Strong communication and empathy skills.
• Teamwork, continuous learning, and high-quality care.
• E-Clinical Works EMR experience is a plus.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties for continuity of care.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when necessary.
• Educate patients and families on treatment plans and prevention.

• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

Requirements

• Certified Nurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C) licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability to connect with patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
• E-Clinical Works EMR experience a PLUS!
