Job summary

Hometown Urgent Care & Occupational Health in Berea, OH is seeking a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive outpatient primary care across the lifespan. You will conduct exams, diagnose, and manage conditions, collaborate with specialists, and educate patients while utilizing E-ClinicalWorks for documentation.

Requirements include Ohio-licensed NP-C/FNP or PA-C, experience in primary care or internal medicine, and strong communication and teamwork skills.

Qualifications

• Licensed NP-C/FNP or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Comfortable managing a broad patient panel across all ages.
• EMR experience with E-ClinicalWorks is a plus.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties for continuity of care.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when necessary.
• Educate patients and families on treatment plans and lifestyle changes.

What You'll Do:

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists in ENT, Surgical, and Medical Subspecialties to ensure continuity and coordination of care
• Utilize E-ClinicalWorks for efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

Requirements

• Certified Nurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C) licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability to connect with patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
• E-Clinical Works EMR experience a PLUS!
