Job summary

Hometown Urgent Care is seeking a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in Ohio. You will work within a collaborative team to diagnose, treat, educate, and prevent illness, while coordinating care with ENT, surgical, and medical subspecialties.

The role requires current Ohio NP-C/FNP or PA-C licensure, primary care experience, and strong communication skills to connect with diverse patients.

Qualifications

• Licensed NP-C or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Ability to manage a broad patient panel across ages.
• Proficient in outpatient clinical procedures.
• Strong communication and empathy in patient care.

Responsibilities

• Perform comprehensive physical exams, histories, and assessments.
• Diagnose and manage a wide range of conditions in primary care.
• Provide care across multiple specialties as needed for continuity.

We are seeking acompassionate and skilled Advanced Practice Provider (NP or PA)to provide comprehensive primary care services to patients across the lifespan. What You’ll Do:

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists inENT, Surgical, and Medical Subspecialtiesto ensure continuity and coordination of care
• UtilizeE-ClinicalWorksfor efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability toconnectwith patients matters as much as your clinical expertise
