Comprehensive Rehab Consultants in Van Wert, OH, seeks a PM&R clinician (NP/PA) to provide rehabilitative services in SNF/post-acute settings. You will collaborate with inter-disciplinary teams to optimize patient outcomes while maintaining autonomous practice in a designated facility.

This 16-hour per week W2 role offers a weekday schedule (8 a.m.–5 p.m.), competitive pay of $75–$80 per hour, and benefits including malpractice coverage, 401(k), CEU credits, and a laptop.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Ohio license required.
• Ability to work in the U.S.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility with strong clinical leadership.
• Collaborate with PCP and therapy teams to manage SNF conditions.
• Monitor patient progress with PT/OT/SLP teams and track therapy outcomes.
• Attend multidisciplinary meetings and support culture building.
