Job summary

Comprehensive Rehab Consultants seeks a Part-time NP/PA with PM&R expertise to provide physiatr y services in designated SNF/post-acute centers in Springfield and Dayton, OH. The clinician will collaborate with multidisciplinary teams to optimize patient outcomes and independence.

Hours hover around 9 AM–4 PM (flexible weekdays). The role includes a 75.00–87.50 USD hourly pay, 8-hour shifts, and benefits such as 401(k), malpractice coverage, CEU access, and a company laptop.

Qualifications

• Nurse Practitioner/APRN/PA license required.
• Ohio license required.
• Experience in post-acute space preferred.
• Confidence to function autonomously.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Represent CRC with facility leadership and interdisciplinary teams.
• Collaborate with PCP teams for management of SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Track patient progress and barriers to therapy; participate in multidisciplinary meetings.
