Job summary

Comprehensive Rehab Consultants is seeking a Part-time PM&R Clinician (Nurse Practitioner/PA) to provide specialized Physiatry services within a designated SNF or post-acute rehab center in Van Wert, OH. You will collaborate with the post-acute therapy team to optimize patient outcomes and promote independence.

The position emphasizes autonomous practice, compassionate geriatric care, and ongoing opportunities for professional growth and leadership within CRC.

Qualifications

• Post-acute/SNF experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute centers.
• Lead and collaborate with interdisciplinary care teams within the facility.
• Assist in management of common post-acute conditions (CVA, neurological conditions, postoperative ortho).
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
