Job summary

Comprehensive Rehab Consultants is seeking a part-time PM&R clinician to provide Physiatry services exclusively at a designated SNF/post-acute rehab center in Van Wert, OH. The role emphasizes autonomy, strong clinical leadership, and collaboration with a cross-disciplinary team.

The ideal candidate will have NP/APRN/PA licensure, Ohio licensure, and post-acute experience. Competitive hourly pay and benefits accompany this position.

Qualifications

Nurse Practitioner/APRN/PA license required.
Ohio license required.
Ability to work in the U.S.
Post-acute/SNF experience preferred.
Strong patient care attitude.

Responsibilities

Provide specialized PM&R/Physiatry services as a rehab consultant.
Represent CRC in your facility through clinical leadership and collaboration with the care team.
Collaborate with PCP teams to manage SNF/post-acute conditions.
Work with PT/OT/SLP teams to improve patient function and quality of life.
Follow therapy patients to track progress in therapy.
Assess and manage barriers to therapy including pain, spasticity, neurogenic issues.
Attend multidisciplinary meetings and bridge therapy with internal medicine.
Engage in culture-building and ongoing education.
