Job summary

Comprehensive Rehab Consultants in Modesto, CA is seeking an NP/PA PM&R Clinician to provide PM&R services at a designated SNF or post-acute rehab center. You will work with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence.

The role offers a 16-hour, part-time schedule (9 AM–4 PM) with California-licensed providers. SNF/post-acute experience preferred, plus opportunities for CRC certification, CEUs, and a laptop. Competitive pay starts at $100/hour.

Qualifications

• Nurse Practitioner/APRN/PA License required and active in CA.
• Post-acute/SNF experience preferred.
• Ability to work autonomously with strong clinical leadership.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Lead the facility team and collaborate with interdisciplinary care teams.
• Coordinate with PCPs for management of common SNF conditions.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Engage in culture building and ongoing education sessions.
