Job summary

Comprehensive Rehab Consultants in Walton Hills, OH is seeking a part-time PM&R Clinician (NP/PA) to provide rehabilitation services at designated SNF or post-acute centers. You will work with the post-acute therapy team and lead clinical care with autonomy.

The role offers an 8-hour day, weekday schedule, competitive hourly pay of $75–$80, 100% malpractice coverage, 401(k), CEU opportunities, and access to physiatry certification while supporting patient independence and quality of life.

Qualifications

• Licensed NP/APRN/PA in Ohio
• Experience in post-acute care settings preferred
• Ability to work autonomously with strong interpersonal skills
• Compassionate and patient-focused care delivery

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant
• Represent CRC in the facility and lead with the interdisciplinary team
• Collaborate with PCPs to manage SNF/post-acute conditions
• Coordinate with PT/OT/SLP for functional improvement
• Monitor patient progress and address barriers to therapy
• Bridge therapy and internal medicine within the SNF team
• Engage in culture-building and ongoing education programs
