Comprehensive Rehab Consultants in Van Wert, OH is seeking a Part-time PM&R Clinician (NP/PA) to provide specialized Physiatry services at a designated SNF. You will collaborate with the post-acute therapy team, PCPs, and facility staff to improve patient function while maintaining a healthy work-life balance.

This role offers autonomy, a weekday schedule, and a competitive hourly rate of $75–$80, plus benefits including malpractice coverage, CEUs, a laptop, and a 401(k) plan.

Qualifications

• Current Nurse Practitioner/APRN/PA license in Ohio.
• Ability to work in the U.S. without sponsorship.
• Experience in post-acute care or SNF setting preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in the facility through strong clinical leadership and collaboration with the interdisciplinary care team.
• Collaborate with PCP teams to manage conditions common in SNFs (e.g., CVA, progressive neurological conditions, postoperative orthopedic surgeries).
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and address barriers to therapy including pain, spasticity, neurogenic bowel/bladder, and more.
• Attend multidisciplinary meetings and contribute to culture building and ongoing education.
