Comprehensive Rehab Consultants (CRC) is seeking a PM&R clinician (NP/PA) to provide specialized Physical Medicine and Rehabilitation services at a designated SNF or post-acute rehab center in Bowling Green, OH. This 16-hour, part-time W2 role offers autonomy and collaboration with the post-acute therapy team to optimize patient outcomes.

The ideal candidate will hold an NP/PA license in Ohio, possess post-acute care experience, and be able to work in the U.S.

Competenze

• Nurse Practitioner/APRN/PA license required
• Ohio license holder
• Ability to work in the U.S.
• Post-acute space experience preferred

Mansioni

• Provide PM&R/Physiatry services as a rehab consultant
• Represent CRC in your facility with strong clinical leadership and collaboration with the interdisciplinary care team
• Collaborate with PCP teams to manage conditions common in SNFs such as CVA and progressive neurological conditions
• Work with PT, OT, and SLP teams to improve patient function and quality of life
• Follow therapy patients to track progress and address barriers to therapy
• Participate in multidisciplinary meetings and CRC culture-building activities
