Job summary

Comprehensive Rehab Consultants in Edinboro, PA is seeking a part-time PM&R clinician to provide specialized Physical Medicine and Rehabilitation services at a designated SNF or post-acute rehab center, collaborating with an interdisciplinary team.

You will partner with PCPs and therapy teams to manage common SNF conditions, while promoting patient independence and optimal outcomes. The role offers a Monday–Friday schedule with competitive hourly pay.

Qualifications

• Nurse Practitioner/APRN license required.
• PA license to practice in the U.S. required.
• Post-acute/SNF experience preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in the facility with clinical leadership.
• Collaborate with PCP teams for SNF-related conditions.
• Work with PT/OT/SLP to improve function and quality of life.
• Follow patients to track progress in therapy.
• Address barriers to therapy and care optimization.
• Attend multidisciplinary meetings and support culture-building.
• Participate in ongoingDidactics and regional team activities.
