Comprehensive Rehab Consultants (CRC) seeks a Part-time PM&R Clinician (NP/PA) to provide specialized Physiatry services at a designated SNF or post-acute rehab center in Walton Hills, OH. You will work with an integrated team to promote patient independence and optimal rehabilitation outcomes.

The role emphasizes autonomy, flexible scheduling, and opportunities for professional development, including in-services and regional training. A driving passion for geriatric post-acute care is essential.

Qualifications

• Post-acute/post-acute care experience preferred.
• Ability to practice autonomously in SNF/post-acute settings.
• Strong interpersonal and communication skills.
• Empathy and compassion for geriatric patients.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in facility with clinical leadership and team collaboration.
• Collaborate with PCPs on management of SNF conditions.
• Coordinate with PT/OT/SLP to improve patient function.
• Track therapy progress and barriers to therapy.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education within the team.
