Job summary

Comprehensive Rehab Consultants in Chico, CA is seeking a NP/PA/Physiatry clinician to provide PM&R services at a SNF or post-acute rehab center. You will collaborate with the post-acute therapy team to improve patient independence and outcomes.

The role offers a 40-hour full-time W-2 schedule with a competitive salary starting at $150,000 per year, plus generous benefits and opportunities for clinical leadership and growth within CRC.

Qualifications

• NP/PA/Physician Assistant licensed in California.
• Experience in post-acute care and SNF settings preferred.
• Ability to work autonomously within an interdisciplinary team.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through clinical leadership and collaboration with the care team.
• Collaborate with PCP teams to manage common SNF conditions (CVA, neurological conditions, postoperative ortho).
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress in therapy.
• Assess and manage barriers to therapy (pain, spasticity, neurogenic issues).
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education including monthly Didactic lectures and regional meetings.
