Job summary

Comprehensive Rehab Consultants (CRC) seeks a NP/PA/APRn PM&R clinician to deliver specialized physiatry services at a designated SNF or post-acute rehab center in Chico, CA.

You will collaborate with PCPs and therapy teams to optimize rehabilitative outcomes, promote patient independence, and navigate post-acute care pathways. The role offers a comprehensive benefits package, education opportunities, and flexible scheduling within a full-time W-2 arrangement.

Qualifications

• California-licensed NP/PA/APRn with ability to practice in the U.S.
• Post-acute/SNF experience preferred.
• Demonstrated autonomy in clinical settings.
• Strong interpersonal skills and patient-focused care.

Responsibilities

• Provide PM&R/Physiatry services in SNF; rehab consultant.
• Represent CRC with strong clinical leadership in the facility.
• Collaborate with PCPs and interprofessional teams.
• Work with PT/OT/SLP to improve function and QoL.
• Monitor patient progress and address therapy barriers.
• Participate in multidisciplinary meetings and culture-building.
