Job summary

Comprehensive Rehab Consultants seeks a PM&R Clinician to deliver specialized Physiatry services at a designated SNF or post-acute rehab center in Washington, NC. You will work within the post-acute therapy team to optimize outcomes and patient independence.

The ideal candidate has NP/PA credentials and NC licensure, with a preference for post-acute/SNF experience. The role offers a full-time schedule, competitive salary, and opportunities for certification and education within CRC.

Qualifications

• Licensed NP/APRN/PA in North Carolina.
• Post-acute care experience preferred.
• Ability to work in the U.S.

Responsibilities

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility through leadership and collaboration with the care team.
• Collaborate with PCP teams to manage SNF/post-acute conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Follow patients to track progress in therapy.
• Address barriers to therapy (pain, spasticity, neurogenic issues).
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
