Comprehensive Rehab Consultants in Modesto, CA is seeking a NP/PA/Physiatry clinician to provide PM&R services at designated SNFs. This full-time role offers autonomy, collaboration with PT/OT/SLP, and leadership opportunities within interdisciplinary teams.

The position features a competitive salary starting at $100,000, comprehensive benefits, and a 8 AM–5 PM schedule with a 3-day work week option. California NP/PA license and post-acute experience are preferred.

Qualifications

• NP/APRN/PA license in California
• Post-acute/SNF experience preferred
• Ability to work autonomously with strong communication

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant
• Represent CRC in your facility with clinical leadership
• Collaborate with PCP teams on SNF conditions (CVA, neurodegenerative, orthopedic surgeries)
• Work with PT/OT/SLP teams to improve function and quality of life
• Track progress of patients in therapy and document outcomes
• Assist in addressing barriers to therapy (pain, spasticity, neurogenic issues)
• Participate in multidisciplinary meetings and bridge therapy with internal medicine
• Support culture-building through didactics and regional meetings
