Job summary

Comprehensive Rehab Consultants is seeking a PM&R Clinician (NP/PA) to provide Physiatry services at designated SNFs in Modesto, CA. You will lead care within the post-acute team, collaborating with PCPs and therapy colleagues to optimize patient independence and outcomes.

Requirements include NP/PA license in California, post-acute experience preferred, and a commitment to ongoing education and clinical leadership within CRC.

Qualifications

• NP/PA/Physician Assistant license required.
• California license to practice in the U.S.
• Experience in post-acute space preferred.
• Great attitude and desire to deliver best patient care.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with strong clinical leadership.
• Collaborate with PCPs to manage SNF conditions like CVA and neurological issues.
• Work with PT/OT/SLP teams to improve patient function.
• Follow therapy patients to track progress in therapy.
• Assist in bridging therapy and internal medicine in the SNF team.
• Participate in ongoing education and regional team activities.
