Comprehensive Rehab Consultants in Chico, CA is hiring a PM&R Clinician (Nurse Practitioner/APRN/PA) to support post-acute care in SNFs. You will work with a multidisciplinary team to optimize rehabilitative outcomes and promote patient independence, with a flexible yet consistent 9am–4pm schedule.

Qualifications

• Nurse Practitioner/APRN/ Physician Assistant qualification.
• California License is required to practice in the state.
• Ability to work in the U.S.
• Post-acute care/ SNF experience preferred.
• Commitment to delivering high-quality patient care.
• 40 hours/week work expectation.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Collaborate with the interdisciplinary care team in SNF/post-acute settings.
• Coordinate care with PCPs to manage conditions common in SNFs.
• Work with therapy teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
