Job summary

Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner, APRN, or PA to provide PM&R/Physiatry services at a designated SNF or post-acute rehab center in Chico, CA. This is a 40-hour, full-time W-2 role with autonomy within a collaborative interdisciplinary team.

You will work with PCPs and rehab staff to manage conditions common in SNFs, promote patient independence, and participate in ongoing education. Starting salary is $150,000 per year.

Qualifications

• Nurse Practitioner/APRN/PA required.
• California license required.
• Experience in post-acute space preferred.
• Ability to work in the U.S.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC with strong clinical leadership in the facility.
• Collaborate with PCP teams to manage common SNF conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
• Engage in culture-building opportunities and ongoing education.
