Job summary

Comprehensive Rehab Consultants (CRC) seeks a Nurse Practitioner/PA with PM&R focus to deliver rehab services at a Chico, CA SNF/post-acute center. You will lead care in collaboration with the interdisciplinary team.

Qualifications

• Licensed Nurse Practitioner, APRN, or PA in California.
• Post-acute/SNF experience preferred.
• Ability to work autonomously in a facility-based role.
• Strong patient care, empathy, and communication skills.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in the facility with the care team.
• Collaborate with PCPs on post-acute conditions.
• Coordinate with PT/OT/SLP to improve function.
• Attend multidisciplinary meetings and promote care continuity.
• Engage in education and culture-building activities.
