Job summary

Comprehensive Rehab Consultants seeks an NP/PA with PM&R to deliver rehabilitative care at a designated SNF/post-acute center in Alamo, TX. The clinician leads care, collaborates with the interdisciplinary team, and focuses on improving patient independence.

This 32-hour W-2 role offers a robust benefits package, education reimbursement, and opportunities for clinical leadership and advancement within CRC.

Qualifications

• Nurse Practitioner/APRN/PA license.
• Texas License.
• Ability to work in the U.S.
• Post-acute/post-acute space experience preferred.
• SNF exposure preferred.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC with clinical leadership in your facility.
• Collaborate with PCP teams for SNF-related conditions (CVA, neurological conditions, post-op ortho).
• Coordinate with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy and internal medicine.
