Job summary

Concord Hospital Health System is seeking a Patient Care Coordinator to support patient registration, referrals, and care coordination. You will serve as the primary access point for patients and families, delivering timely, efficient, and compassionate service.

Under supervision of Practice Administration or Assistant Practice Manager, you will manage scheduling, enrollment, EMR data, and communications while promoting a patient-centered medical home.

Qualifications

• High School diploma or GED required.
• Experience in customer service is preferred.
• Knowledge of medical office operations: scheduling, registration, EMR, referrals, authorizations, payer guidelines, compliance.
• Familiarity with medical terminology preferred.

Responsibilities

• Greets and assists patients; secures signatures for required documentation; follows CHMG policies.
• Verifies insurance and demographic information; makes collection attempts and educates on resources; reconciles lists.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information and maintains medical records.
• Processes and routes incoming calls and responds to inquiries.
• Schedules and reschedules appointments; promotes patient portal enrollment.
• Distributes mail, faxes and electronic items; educates patients on form completion.
• Supports a culture of patient-centered care and works on team and solo projects.
