Job summary

Concord Hospital is seeking a Patient Care Coordinator to support patient registration, referral coordination, and front-desk phone management under supervision of Practice Administration.

The PCC will greet patients, verify insurance, schedule appointments, coordinate intake, maintain medical records, and support a patient-centered medical home culture; strong customer service and medical office knowledge are essential.

Qualifications

• High School diploma or GED required; annual registration exam may be required.
• Demonstrated customer service ability with the public, staff and patients preferred.
• Knowledge of medical office operations and patient flow is expected.

Responsibilities

• Greets and assists patients; obtains signatures for documentation; follows policies.
• Verifies insurance and demographic information; conducts collection attempts with patient education.
• Reconciles work lists and reports; coordinates intake and maintains medical records.
• Schedules, confirms or reschedules appointments; handles phone calls and inquiries.
• Promotes enrollment in patient resources and supports a patient-centered medical home.
• Works independently and with team on projects and information flows.
