Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. You will provide PT services under supervision, perform screenings, and help implement treatment plans to improve residents' mobility and function.

The role requires an accredited PTA program degree, an active PTA license, and ongoing professional development. CMS-compliant documentation in NetHealth EMR is expected, with opportunities for growth within the organization.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under the Physical Therapist.
• Deliver individual and group treatment addressing strength, mobility, gait, and ROM.
• Monitor resident responses and modify treatment as indicated by the Physical Therapist.
• Document CMS-compliant and medically necessary care in NetHealth EMR.
• Participate in resident, family, and staff education.
