Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. The PA will support the Physical Therapist, conduct screenings, and help implement treatment plans to promote resident mobility and independence.

Full Time, Part Time, or PRN options are available. Candidates must have an Associate's degree in Physical Therapy, an active PTA license, and commitment to ongoing education as CMS documentation is required in NetHealth EMR.

Qualifications

• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Ability to document per CMS expectations in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluating and setting short- and long-term goals and treatment plans.
• Assist in selecting and sequencing PT techniques and activities.
• Monitor residents’ response and modify treatment as indicated.
• Assist in coordinating resident schedules and transporting schedules with the PT and team.
