Evolve Therapy Services is hiring a Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Freeport, IL. You will provide therapy services under the supervision of a Physical Therapist and help implement treatment plans and goals.

The role involves resident screenings, individual and group treatments, and documentation in NetHealth EMR to meet CMS requirements. Requires an active PTA license and ongoing continuing education; multiple shift options available.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist the Physical Therapist in identifying target goals and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan.
• Monitor residents’ response to therapy and modify as indicated; document in EMR.
