Job summary

Evolve Therapy Services in Freeport, IL is seeking a compassionate Physical Therapy Assistant to join an in-house Skilled Nursing Facility. The role supports a Physical Therapist and delivers patient care under supervision, with flexible full-time, part-time, or PRN scheduling.

You will conduct screenings, assist in evaluations and goal setting, administer individual and group therapy, document in NetHealth CMS-compliant records, and participate in resident and family education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying target goals and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan.
• Document compliant CMS- required records in NetHealth EMR systems.
• Train resident, family and appropriate staff in equipment use.
