Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. The PTA assists the Physical Therapist, conducts screenings, and helps implement treatment plans to improve residents’ mobility and function.

Eligible candidates hold an Associate's degree in physical therapy, an active PTA license in the state, and commit to ongoing continuing education. Full time, part time, or PRN positions available.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and development of treatment plans with the Physical Therapist.
• Implement treatment plans including strength, mobility, balance, gait, and ROM activities.
• Monitor resident response and adjust interventions in consultation with the PT.
• Complete compliant CMS documentation in NetHealth EMR systems.
