Evolve Therapy Services is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Illinois. The role involves providing therapy under a supervising PT, conducting screenings, and contributing to individualized treatment plans.

Requirements include an Associate's degree in Physical Therapy and an active PTA license in the applicable state, with ongoing continuing education to maintain licensure.

Qualifications

• Associate's degree in Physical Therapy required.
• Active Physical Therapist Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct both individual and group treatment as outlined in the treatment plan.
• Monitor residents’ response to intervention and consult with the Physical Therapist as needed.
• Assist in establishing and modifying resident schedules with the Physical Therapist and team.
• Train residents and staff in the use of equipment.
• Complete compliant and medically necessary documentation in NetHealth EMR.
