Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility partner in Freeport, IL. The PTA will work under the supervision of a Physical Therapist to deliver and adapt therapy plans, perform screenings, and participate in resident education.

The role requires an Associate's degree in physical therapy and an active PTA license, with ongoing professional development to maintain licensure.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist in evaluation and developing target goals and treatment plans with the Physical Therapist.
• Assist in identifying and sequencing therapy activities and adapting techniques as indicated.
• Monitor residents’ response to intervention and communicate modifications to the Physical Therapist.
• Complete CMS-compliant documentation in NetHealth EMR.
