Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. The role requires supporting the Physical Therapist, conducting screenings and implementing treatment plans to improve resident mobility and function.

Applicants must have an Associate degree in Physical Therapy and an active PTA license. Ongoing education is expected to maintain licensure and certification; benefits offered for full-time status.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluating and plan development with the Physical Therapist.
• Support treatment plans including strength, mobility, gait, and ROM activities.
• Educate residents and families on therapy techniques and equipment.
