Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility located in Freeport, IL. Under the supervision of a Physical Therapist, you will provide PT services and participate in screenings, evaluations, and treatment planning to ensure high-quality resident care.

Responsibilities include individual and group therapy, treatment planning, and education for residents, families, and staff.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in identifying a target list with short- and long-term goals and treatment plan.
• Conduct individual and group treatments including muscle strength, mobility, balance, gait, ROM and modalities.
• Assist in selecting PT techniques/media and adapt as indicated.
• Monitor residents’ responses and modify care in collaboration with the PT.
• Assist in scheduling and coordinating resident therapies with the team.
• Train residents, family, and staff on equipment use and safety.
• Participate in education of residents and families; ensure documentation per CMS and NetHealth EMR.
