Job summary

Flagstar Rehab is seeking a skilled Physical Therapy Assistant (PTA) to join our team in Skilled Nursing Facilities. Active New York PTA license required. Flexible schedule with full-time, part-time, and per diem opportunities, plus per-diem benefits and strong support.

You will work under PT supervision, follow care plans, and help patients with TBI, stroke, Parkinson’s, and orthopedic needs in SNFs. Competitive compensation and easy access to support are offered.

• BLS Certification required.
• EMR systems proficiency expected.

Responsibilities

• Collaborate with PT Supervisor and follow the established Plan of Care for all patients.

Hourly Compensation:

• 1099: $50

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
