Job summary

Five Star Nursing seeks Physical Therapy Assistants (PTAs) to support Skilled Nursing Facilities with flexible scheduling and a strong support system. Per diem and independent contracting options are available, with LLC guidance provided.

You’ll experience clear communication, quick placement, and a team that’s accessible for support. Compensation is listed as $34–$36/hour (W2) or $38–$40/hour (1099), with weekly pay and benefits where eligible.

Qualifications

• Current NYS PTA license and registration.
• Valid BLS certification.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress.
