Job summary

Flagstar Rehab seeks a licensed Physical Therapy Assistant (PTA) to join our rehab department in Skilled Nursing Facilities across NYC and surrounding areas.

As a PTA, you will work with a multidisciplinary team to deliver treatment, educate patients and families, and support functional improvements. EMR documentation and adherence to care plans are essential. We offer both W-2 and 1099 options and flexible schedules.

Qualifications

• Current NYS PTA license and registration is required.
• EMR competence is required to monitor and document patient progress.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/liability insurance (if applicable) may be required.

Responsibilities

• Collaborate with PT Supervisor and follow the established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting for various conditions.
• Monitor and document patient progress using EMR software.

Flagstar Rehab PTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Job Duties

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
