Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) to join our network of Skilled Nursing Facilities. This role focuses on delivering comprehensive therapy under the supervision of a PT, with flexible scheduling and opportunities to manage your own case flow within SNF settings.

We offer a choose-the-way-you-earn model, quick interviews, and remote onboarding to help you start smoothly. Licensure in NYS, BLS, and strong EMR skills are essential for success.

Qualifications

• Must hold current NYS PTA license and registration.
• BLS certification required.
• EMR competence required.
• Must meet all State and Federal employment requirements for US nursing facility.
• Malpractice/liability insurance if applicable.

Responsibilities

• Collaborate with PT supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to SNF patients with various conditions.
• Monitor and document patient progress.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
