Job summary

Flagstar Rehab is seeking passionate Physical Therapy Assistants (PTAs) for opportunities in Skilled Nursing Facilities. You’ll join a supportive team with flexible schedules, per diem options, and independent contracting guidance.

Active NY PTA license required; duties include following Plan of Care, delivering therapy, and monitoring patient progress in SNF settings. Weekly pay and comprehensive benefits are available for eligible roles.

Qualifications

• Current NYS PTA license and registration.
• Valid BLS certification.
• EMR competence and ability to document progress.
• Meets state and federal employment requirements for US nursing facility.
• Malpractice/liability insurance if applicable.

Responsibilities

• Collaborate with PT Supervisor and follow Plan of Care for all patients.
• Provide quality therapy services to SNF patients with various conditions.
• Monitor and document patient progress.

Hourly Compensation:

• 1099: $50

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
