Job summary

Flagstar Rehab is seeking Physical Therapy Assistants (PTAs) to join our Skilled Nursing Facility teams in the New York area. You will collaborate with a multidisciplinary team to deliver treatments and patient education to improve function and cognition, enabling daily activities.

The role offers flexible schedules across full-time, part-time, per diem, evenings and weekends. We provide direct employment or independent contractor options, plus weekly pay and support services.

Qualifications

• Current NYS PTA License and Registration.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Collaborate with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to SNF residents with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery.
• Monitor and document patient progress using EMR software.

Physical Therapy Assistant Job Duties

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
