SwedishAmerican Hospital in Rockford, IL is seeking a licensed Physical Therapist for a hybrid role. Work Monday–Friday 8:00am–5:00pm with in-person duties at 2550 Charles St. A sign-on bonus of up to $5,000 may be available to eligible applicants.

The position includes benefits and opportunities for professional development, with emphasis on delivering high-quality patient care and collaboration with an interdisciplinary team. Illinois licensure and CPR certification are required.

Work Schedule:

80% FTE, Monday-Friday 8:00am-5:00pm this is a hybrid position, in person work location is 2550 Charles St. You may be eligible for up to a $5,000 sign on bonus.

Additional components of compensation may include:

• Evening, night, and weekend shift differential

Qualifications

• Completion of an accredited program approved by the American Physical Therapy Association. Required

Licenses & Certifications

• Current license from the State of Illinois or in the process of acquiring licensure. Required
• Must possess a current, valid driver's license issued by the individual's state of residence. Required
• Basic Life Support/CPR Certification. Required
• An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
