Job summary

UW Health Home Health in northern Illinois is seeking a licensed Occupational Therapist to provide patient-centered care in the home setting. This 100% full-time role involves evaluating functional abilities, developing treatment plans, and delivering skilled interventions to support independence.

Qualifications

• Must be a graduate of an accredited Occupational Therapy program.
• Current license from the State of Illinois or in the process of acquiring licensure. Required
• Must possess a current, valid driver's license issued by the individual's state of residence. Required
• Basic Life Support/CPR Certification. Required

Job description

Work Schedule:

100% FTE, Sunday-Saturday 8:00am-5:00pm, you will work at UW Health Home Health 2550 Charles St.

Additional components of compensation may include:

• Evening, night, and weekend shift differential

Qualifications
