Job summary

UW Health in northern Illinois seeks a licensed Physical Therapist for a hybrid role based in Rockford, IL. The position is 80% FTE, Monday–Friday 8:00am–5:00pm with in-person work at 2550 Charles St. A sign-on bonus of up to $5,000 is available.

Qualifications

• Completion of an accredited program approved by the American Physical Therapy Association.
• Current Illinois licensure or in process.
• Valid driver's license required.
• BLS/CPR certification required.
• Evidence of licensure eligibility to practice must be provided at employment time.

Qualifications

• Completion of an accredited program approved by the American Physical Therapy Association. Required

Licenses & Certifications

• Current license from the State of Illinois or in the process of acquiring licensure. Required
• Must possess a current, valid driver's license issued by the individual's state of residence. Required
• Basic Life Support/CPR Certification. Required
• An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
