Job summary

UW Health in northern Illinois seeks a per diem Physical Therapist to join our Home Health team. The role offers varied scheduling with Sunday–Saturday hours and a focus on delivering high-quality in-home care.

You will work in a supportive environment with opportunities for differential pay in evenings, nights, weekends, and on-call scenarios, along with comprehensive benefits and pathways for professional development.

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Participates in educational programs on an ongoing basis and completes mandatory CEUs.
• Current Illinois licensure or eligibility and ability to provide documentation at start.

Work Schedule

Per Diem, varied schedule, Sunday-Saturday 0800-1700. You will work at the UW Health Home Health department.

Additional components of compensation may include:

• Evening, night, and weekend shift differential

Qualifications

• Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Required
• Participates in educational programs on an ongoing basis and complete mandatory CEUs. Required

Licenses & Certifications

• Current license from the State of Illinois or in the process of acquiring licensure. An applicant for employment as a physical, occupational, or speech therapist must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist until evidence of licensure eligibility is provided. Required
• Basic Life Support/CPR Certification. Required
