Job summary

UW Health SwedishAmerican in Rockford, IL is seeking a licensed therapist for an 80% FTE hybrid position. Hours are Monday–Friday, 8:00am–5:00pm, with in-person work at 2550 Charles St. A sign-on bonus of up to $5,000 may be available.

The role requires graduation from an accredited program, state licensure or eligibility in Illinois, CPR/BLS certification, and a valid driver's license. There are strong benefits and opportunities for professional growth.

Qualifications

• Completion of an accredited program approved by the American Physical Therapy Association. Required
• Current license from the State of Illinois or in the process of acquiring licensure. Required
• Must possess a current, valid driver’s license issued by the individual’s state of residence. Required
• Basic Life Support/CPR Certification. Required
• An applicant for employment as a physical, occupational, or speech therapist must present licensure evidence at time of employment. Required
