Job summary

UW Health SwedishAmerican is seeking a licensed Physical Therapist at the UW Health 2550 Charles St Clinic in northern Illinois. This 100% FTE, day shift role runs Monday–Friday 8:00 a.m.–4:30 p.m., with every third Saturday 8:00 a.m.–4:30 p.m. and includes a $20,000 sign-on bonus for eligible candidates.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Current Illinois PT license required.
• BLS/CPR certification required.
• Valid state driver’s license and proof of liability insurance required.

Responsibilities

• Provide prescribed physical therapy treatments and ensure patient care quality.
• Collaborate with healthcare team to optimize patient outcomes.
• Document patient progress and adjust treatment plans as needed.

Job Description

Additional components of compensation may include:

• Evening, night, and weekend shift differential
• Overtime

Qualifications

• Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Required

Licenses & Certifications

• Current license from the State of Illinois. Required
• Basic Life Support/CPR Certification. Required
• Valid State driver’s license and proof of liability insurance. Required
• An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
