Job summary

UW Health Swedish American Hospital in Rockford, IL is seeking a licensed Physical Therapist to join our team. This 100% FTE role features Monday–Friday hours with variable scheduling and will be based at 1401 E State St.

You will participate in ongoing CEUs, maintain licensure, and contribute to a compassionate, patient-centered care environment. UW Health offers a comprehensive benefits package, including PTO, medical, dental, vision, retirement, and tuition assistance.

• Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Required

Work Schedule

100% FTE, Monday-Friday variable hours, you will work at UW Health Swedish American Hospital 1401 E State St.

Licenses & Certifications

• Current license from the State of Illinois or in the process of acquiring licensure. An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
• CPR certification. Required
