Evolve Therapy Services, LLC in Hopkins, MN is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. This full-time or PRN role supports the Physical Therapist in delivering resident-centered care.

You will perform screenings, assist in evaluations, create treatment plans, and guide group and individual therapy sessions, while documenting in NetHealth EMR and coordinating with the care team.

Formación

• Associate degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under guidance of the Physical Therapist.
• Conduct both individual and group treatment as outlined in the treatment plan.
• Document CMS-required progress in NetHealth EMR and coordinate with the care team.
