Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. Full Time, Part Time or PRN positions available. You will support the Physical Therapist to implement and monitor treatment plans for residents.

Responsibilities include screenings, group and individual therapy, documentation in NetHealth EMR, and collaboration with the care team to meet resident goals.

Qualifications

• Licensed PTA with current licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under supervision.
• Conduct screenings to determine need for intervention.
• Assist in evaluation planning with the Physical Therapist.
• Develop and modify treatment plans and goals.
• Lead individual and group therapy sessions per plan.
• Document compliant data in NetHealth EMR.
• Coordinate schedules with the PT and care team.
• Educate residents, families and staff on exercises.
• Ensure timely and productive shift performance.
